Covid Age Toolkit (demo)

This online form will help you identify if you are more likely to suffer from serious illness if you contract Covid-19. The outcome of the assessment will provide you with your risk rating based on your 'Covid-age'.

Please note this tool will not predict your likelihood of becoming infected with coronavirus or passing it to others.

If you have a 'shielding letter', symptoms of an underlying health concern or a diagnosis not mentioned in this form, you must always follow the advice of your GP or specialist.

Clinical judgement must be used in conjunction with the Covid-age toolkit. Covid-age is to aid discussion around risk and vulnerability but is not an exact measure.

Regardless of your Covid-age you should continue to follow government rules on social distancing, personal hygiene, and travel.

If you are experiencing symptoms of Covid-19 such as a fever, cough or loss of taste or smell you must immediately self-isolate and contact NHS 111 for advice.
